Drugs:
The Bean Saver database includes 143 antimicrobial agents; including 67 antibacterials, 11 antifungals, 10 antimycobacterials, 5 antiparasitics, 18 antivirals, 32 HIV antiretrovirals, and 5 extended infusion antibiotics. The drugs are listed under their drug class.
Renal Insufficency/ Hemodialysis/ CRRT/ CAPD:
The app provides specific antimicrobial dosage and frequency information for patients with normal renal function and renal insufficiency, as well as for patients on hemodialysis, continuous renal replacement therapy (CRRT), and peritoneal dialysis (CAPD). Bean Saver also provides specific dosing recommendations for patients weighing greater than 150 kg.
Cockroft-Gault Equation:
Antimicrobial dosage adjustments for renal dysfunction are based on estimates of creatinine clearance (CrCl), which reflect the glomerular filtration rate. Bean Saver uses the Cockroft-Gault equation to calculate the CrCl. The patient’s ideal body weight (IBW) is used for this calculation except if the patient’s total body weight (TBW) is > 120% IBW then adjusted body weight (adjBW) is used to calculate CrCl.
Data Entry:
Data entry is quick and easy using either up/down arrows or a custom calculator keypad to enter specific numeric values. Units for height, weight, and serum creatinine can be changed from metric to US units by pushing the specific unit button. All data is entered on one screen in a calculator style, not a wizard style.
Calculate Button:
Once all data fields have been filled and a renal dysfunction selection made, the antimicrobial dosage and frequency will be provided after pushing the “calculate” button. In order to make another calculation the reset button can be pushed and all data fields re-entered or individual data fields can be changed and the “calculate” button pushed for new results.
Weight Based Dosing:
Bean Saver will calculate the appropriate dosage for weight based drugs using total body weight (TBW), ideal body weight (IBW), or adjusted body weight (adjBW) when indicated. For example, instead of simply providing 4-6mg/kg, the dosage will be computed with the original dosage in parathesis.
Settings:
The settings tab allows the user to select the following default settings: antimicrobial generic name vs. trade name, inches vs. centimeters for height, pounds vs. kilograms for weight, and mg/dL vs. umol/L for serum creatinine.
